Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One caregiver had a total of 14 children with a specified age group of two's. Note this was corrected when another caregiver arrived and ratio was distributed and corrected.”
“Last annual sanitation inspection was conducted on 9/15/2023.”
“A 17 month year old was combine with 27 months up to 4 year olds. Note this was corrected when the 17 month old was transferred to the infant room.”
“Two training cups were not labeled with child's name.”
“There were six children supervised by one caregiver, with a specified age of 13 months. There were 15 children supervised by one caregiver, with specified age group of 2 year olds. This situation was corrected when a caregiver called a parent, who then came to pick up two infants. The other parents were also called to pick up 4 toddlers.”
“Two groups had one caregiver each during naptime, however there was no additional caregiver in the building as required.”
“Children did not wash their hands prior to eating lunch.”
“A caregiver was out of ratio when they were left to supervise 10 children including 3 infants during naptime. Due to the number of infants present, the caregiver could only care for up to 7 children. This was corrected when the caregiver combined groups with another caregiver and a total of 12 children were being supervised by 2 caregivers.”
“Based off the DFPS investigation, a caregiver was found reason to believe for neglectful supervision for having a controlled substance in her possession which fell out of her pocket while she was caring for children.”
“Based on the DFPS investigation, the primary caregiver of four infants had a controlled substance in her possession which was accessible to children. The controlled substance was disposed of instead of reporting to Law Enforcement.”
“While reviewing video footage, a caregiver was observed to be on their phone.”
“All children present at the operation including the staff?s children are to be included in ratio and must belong in a classroom. It was determined that a staff member's son was present and not being counted in ratio and allowed to go from classroom to classroom.”
“The director's son was allowed to wander from classroom to classroom which gave him the opportunity to kiss a three-year-old child as she was leaving the operation. It was reported that this occurred more than once, and the situation was not rectified. Minimum standards were not being followed.”
“The fire extinguishers in the operation were last serviced in March 2022.”
“There were toys observed to be in the hand washing sink in the infant area. Note: This was corrected at the inspection when the toys were removed from the sink.”
“A caregiver was observed to be doing administrative duties during the inspection.”
“A caregiver was not aware of the ages of the children she was caring for at the time of the inspection.”
“It was observed that there was an infant left in a playpen for a approximately 30 minutes.”
“The toddler classroom was observed to be lacking defined play centers and age-appropriate toys.”
“A caregiver was observed to walk out of the infant room which resulted in the classroom being left out of ratio.”
“The last maintenance checklist was completed in February 2023. Note: This was corrected at the inspection when the maintenance checklist was update.”
“A caregiver was observed to be overwhelmed and yelling at an infant.”
“A sink in the toddler room used in the diaper changing area was not in working condition. Note: This was corrected at the inspection when water was added to the sink to make it usable for hand washing after diaper changing.”
