Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the inspection, outlets accessible to children were lacking outlet covers in two classrooms.”
“This standard was evaluated during the investigation and was found to be deficient. The operation did not notify the parents of a child who had been left in a classroom immediately after the incident occurred. The operation informed the child's parents at pick up, which was 52 minutes after the incident occurred.”
“This standard was evaluated during the investigation and was found to be deficient. A child was left in a classroom by two caregivers unsupervised for more than five minutes.”
“This standard was evaluated during the investigation and was found to be deficient. The operation did not share an incident report with the child's parents until four days after the incident occurred.”
“A child in care was left unsupervised in a hallway at the operation.”
“Based on interviews with the staff and the parent of the victim, the staff left a child alone unsupervised in the gym. The child left the gym and was observed running down the hall by a collateral witness.”
“This standard was evaluated as part of the investigation and was found to be deficient. On 7/30/2025, a caregiver recorded on an infant daily report that an infant took 4 ounces of milk when the infant did not take their bottle at the feeding; the caregiver did not communicate on the daily report that the infant did not take the milk.”
“During the investigation inspection, a new employee was in the classroom with the children. The person had a pending status for their background check. This was corrected at inspection when the person left the classroom and the operation.”
“This standard was evaluated during the investigation and was found to be deficient. A caregiver patted a child forcefully on the back during nap time as a form of punishment.”
“This standard was evaluated during the investigation and was found to be deficient. Positive methods of discipline and guidance were not used during an occurrence of a child needing redirection.”
“This standard was evaluated during the investigation and was found to be deficient. A caregiver pulled a child by the arms up aggressively while getting to the child up from a nap mat, then pulled the child's arm aggressively again to get the child to come with them.”
“A child did not have immunization records on file.”
“The standard was evaluated with regards to a standard investigation and was found not in compliance the Minimum Standards for Licensed Childcare Centers. Based on interviews with staff, a caregiver in the 3 to 5 years old class did not use positive statements to redirect a child's behavior but instead the caregiver placed the child outside of the classroom with the door closed.”
“The standard was evaluated with regards to a standard investigation and was found not in compliance the Minimum Standards for Licensed Childcare Centers. Based on interviews with staff, a caregiver in the 3- to 5-year-old class yelled at children out of frustration.”
