A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“The netting around the small trampoline in the playroom had rips and tears in it. This was corrected when the entrance of the trampoline net was turned to be facing a corner and not accessible to children. The primary caregiver will take the trampoline apart to remove it.”
