A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“5 staff files were evaluated and all 5 did not have the required affidavit on file at the time of inspection.”
“6 staff no longer employed for over a month have not been inactivate. CORRECTED AT INSPECTION as staff no longer employed were inactivated at the time of inspection.”
