Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“It was determined that a caregiver hit the hand of a child during lunch time in the 12?17-month-old room, this resulted in the caregiver being fired immediately by the director after reviewing video footage.”
“It was determined that during the DFPS investigation a child was physically mistreated on video by a caregiver.”
“The operation allowed two caregivers who did not meet the minimum age requirement to be counted in the caregiver to child ratio.”
“Six of the eight infant's feeding instruction wasn't updated within thirty days.”
“The operation failed to notify licensing that the restrooms are no longer useable.”
“The director did not ensure that the operation maintain compliance with the minimum standards. The director was aware that a child placed an electric cord in his mouth. The director failed to notify licensing and the parents about the incident.”
“One child was observed sleeping in a restrictive device. The caregiver can be observed sitting by the child using her cell phone.”
“A caregiver was not properly supervising children while using her cellphone for personal use which led to a child placing an electric cord in his mouth. Another caregiver was observed using her cellphone while an infant was sleeping in a restrictive device.”
“The operation didn't provide the parents with a copy of the incident report when they child placed an electric cord in his mouth.”
“The operation did not notify a parent that her child placed an electric cord in his mouth.”
