Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff, it was determined a caregiver was not competent in their job duties when they gave a bottle of breast milk to the wrong infant.”
“Two of the ten children records reviewed were missing health statements.”
“During the investigation inspection two caregivers who did not have active background checks were present and were observed caring for children.”
“This standard was evaluated during the investigation and was found to be deficient. A toddler left the group who was outside without the caregivers being aware. The toddler was found unsupervised in a restroom located inside of a classroom by another staff member.”
“In a Pre-K classroom, children were resting on cots that did not have enough walking space for children and caregivers to safely evacuate during an emergency. Photos were taken.”
“This standard was tasked over for this investigation and found to be deficient. The caregiver at the time of the bottle mix-up was not familiar with the child and did not know the name of the baby being fed. The center has a system in place with the infant caregivers to ensure that a name to face is conducted when bottle feeding. Call out the name on the bottle, identify the child, pick up the child, and match the bottle and child together.”
“The written operational policies did not list procedures for provide inclusive services to children with special needs.”
“The operational policies did not list procedures for unassigned Epipens.”
