Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation's director has not ensured required inspections for the operation from third-party entities are being completed timely.”
“The operation's permit holder has not ensured the operation is complying with applicable laws and rules based on the operation's continuous non-compliance and recitations of applicable minimum standards.”
“The operation hasn't had a sanitation inspection completed in 16 months.”
“The operation does not have a current annual fire inspection. The fire inspection expired on 10/1/2024.”
“The operation hasn't had an annual fire inspection completed by a state or local fire marshal. The last inspection completed was about 22 months ago.”
“The operation hasn't had a sanitation inspection completed in a little over 14 months.”
“The operation's permit holder has not ensured the operation is complying with applicable laws and rules based on the operation's continuous non-compliance and recitations of applicable minimum standards.”
“The operation's director has not ensured required inspections for the operation from third-party entities are being completed timely.”
“Children have been fed meals not comparable with the Child and Adult Care Food Program meal patterns.”
“The operation's director has not ensured required inspections for the operation from third-party entities are being completed timely.”
“The operation hasn't had an annual fire inspection completed by a state or local fire marshal.”
“Two previous employees' background checks weren't inactivated within the required time frame.”
“During an inspection at the operation, the young infant classroom was observed to be out of ratio with only one caregiver with 5 infants aged 2 months-9 months in one classroom. This was corrected when one child was moved to the older infant classroom to ensure proper ratio.”
“A known allegation of inappropriate discipline was not reported to Licensing via the proper channel.”
“A child in care and an employee's records were not at the operation and available for review.”
“The allergy plan for a child with allergies wasn't signed by a medical professional.”
“The operation hasn't had an annual fire inspection completed by a state or local fire marshal.”
“It was observed to be one caregiver caring for 6 infants aged 8 months to 14 months. This was corrected by a second caregiver coming in to work in the infant classroom.”
“The operation still hasn't had an annual fire inspection completed by a state or local fire marshal.”
“The operation is delinquent in paying its franchise tax and the right to transact business in Texas has been forfeited.”
“Four out of four employee records reviewed did not have an indicated start date for each employee. Corrected by hire dates being written in files.”
“The operation did not have a current fire inspection completed by a state or local fire marshal. The operation had a current fire alarm system inspection completed by an individual company instead.”
“I observed a child under 12 months sleeping on an activity mat in the infant classroom.”
“The operations fire inspection expired on 10/1/2025.”
“There was an infant without a labeled crib.”
“I observed a caregiver with a provisionally eligible background check caring for a child alone. This was corrected when the director had another staff member without conditions accompany the caregiver with conditions in the classroom.”
“I observed a 2 month old in a crib with a bib that was not on the child's neck and a pacifier clip on a string with beads. This was corrected when the caregiver removed the items from the crib.”
“A caregiver provided the incorrect age of a child. This was corrected when the director informed the caregiver the correct age of the child.”
“Based on information obtained throughout the investigation inappropriate discipline was practiced when a caregiver used physically discipline.”
“Caregivers did not provide the correct ages of the children they were caring for when asked.”
“There were three people in the building without active background checks. One of the people without an active background check left the operation.”
“There was no documentation of the smoke detectors being tested.”
