Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

“Two tubes of diaper rash ointment were observed stored in a basket that was on the floor in the children's restroom located in the toddler room. Note: This was CORRECTED at inspection as the diaper changing supplies were stored in an area out of children's reach.”

“The activity plan posted in the infant room was not current. The activity plan had dates from 4/21/2025 - 4/25/2025.”
“An assigned crib in the infant room was observed with an additional mattress. Note: This was CORRECTED during the inspection as the additional mattress was removed from the crib.”
“An infant between 0-11 months of age was observed asleep in a restrictive device (swing) with a blanket for an undetermined amount of time. Note: This was addressed with the caregiver during inspection and the infant was removed from the swing.”
