Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver did not have shaken baby/ SIDS/brain development training available for review. The caregiver was observed working with infants during inspection.”
“The operation does not have a current children's products certification. The last certification was completed and signed on 2/1/25.”
“A child's admission form is not signed by the child's parent.”
“Director does not have current training in pediatric first aid with rescue breathing. Director's first aid card expired on 2/1/26.”
“An infant child's file is missing updated infant feeding instructions. Instructions were last updated March of 2026.”
“The operation has an electronic carbon monoxide/smoke detection system connected to a fire alarm panel. The panel has not been inspected for over a year. It was last inspected on 10/24/24.”
“During inspection, one caregiver was observed supervising a group of 12 children with a specified age of 18 months. The most number of children one caregiver can supervise for this age group is 9. Therefore, the caregiver was over ratio by 3 children. This was corrected at inspection after a second caregiver arrived to assist with covering ratio.”
“A child's file is missing the health care professional statement. The statement was not available for review.”
