Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A child was left outside for up to three minutes.”
“Two children's files did not have immunization records in them.”
“Seven staff were delinquent in training records from the previous calendar year.”
“One staff member had a First Aid card that expired on 8/15/2026.”
“The menus posted were from June 2026. The cook had menus dating from January 2026 to June 2026. There were no menus printed and posted for July and August 2026. The cook was using the week and the day to determine what food was going to be served. Today is Thursday, the items on the menu for today were listed on a Tuesday.”
“In the Pre-K/Multi-age classroom there were 12 children counted and the caregiver stated there were 11 children at first.”
“A piece of banana that was over two inches long was lying on the floor near the door of the Infant A classroom. It was observed not to be cut into slices, and the caregiver threw the piece of banana in the garbage can.”
“In two children's files there was no addresses provided for the emergency contact person.”
“One caregiver did not have current CPR training. Her CPR card expired on 8/15/26.”
“Two of the children's files did not have the emergency medical authorization completed.”
“During the investigation, it was determined that the operation failed to comply with minimum standards by not providing immunization records to the health department within the requested time frame.”
“An employee in the toddler room was in direct care of children prior to having an eligible background check.”
“One out of ten staff files was missing the pre-affidavit form signed by a caregiver.”
“The operation's fire inspection was past due. The last fire inspection was conducted on 3/4/2024 and was due 3/4/2025.”
“Two of ten children's records reviewed did not have updated immunization records on file.”
“Seven of ten children's records reviewed did not have updated immunization records.”
“Two of ten children's records did not have a health care provider statement on file.”
“One of ten caregivers reviewed for CPR training did not have hands on skills training completed for CPR.”
“A child's nebulizer was stored in the child's cubby and then moved into the closet causing the operation to lose its whereabouts. Due to the medication not being stored properly, the operation is being cited for judgment.”
“A child's nebulizer was stored in the child's cubby and later placed in the closet in the room and not stored with the other medications that are located in the office of the assistant director.”
“A written statement written by the employee was received stating that she "grabbed and patted" the child on the bottom.”
