Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“2 infants in 2 seperate classrooms were seen sleeping in restrictive devices. 1 infant was asleep in a bouncer and 1 infant was asleep in a rocker. This was corrected at inspection by having the caregivers move the infants to their cribs.”
“During the monitoring visit, the operation had one expired medication in the medication container for children.”
“During the monitoring visit, the operational policy book for parents did not have the duration for outdoor play.”
“Four caregivers did not complete the pre-employment affidavit form.”
“This standard was evaluated with regard to an investigation, and a violation was found. The operation did not document the bottle feedings for the victim on the ProCare app for the parent to review the daily feeding for the victim.”
“During the monitoring visit, there were seven children with food allergies. The food allergy form did not have a physician signature.”
“During the monitoring visit, the wooden logs on the playground had nails sticking out, and there appeared to be something growing in between the wooden logs on the playground.”
“During the monitoring visit, I reviewed five staff training certificates for CPR. The class was administered on line.”
“During the monitoring visit, the playground mulch was low and did not meet the depth requirements surrounding the landing area of the slide, and the enclosed playground equipment did not have enough mulch surrounding the equipment.”
“During the monitoring visit, there were three children who did not have up-to-date immunization records available for review during the monitoring visit.”
“There was an 8-month-old infant sleeping in a swing buckled in during the monitoring visit. The infant was removed from the swing during the monitoring visit upon request.”
“A desk fell on a child while in care. The desk was not securely anchored to prevent falling and causing injury to the child.”
“I observed the caregiver not washing the child's hands after changing the childs diaper. This was corrected at inspection when the caregiver washed the childs hands.”
“The operation did not have the children's food allergy plans posted in the kitchen where they prepare food.”
“A caregiver threatened corporal punishment while interacting with a child in care.”
“A caregiver used prohibited punishment when a child was shaken by the shoulders while in care.”
“The provider threatened use of corporal punishment as a form of discipline while interacting with a child.”
“At the time of inspection the wood mulch on both playgrounds was not maintained at the required six inches around the play equipment.”
