Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two immunization records were not current.”
“Based on information obtained the operation did not share incident reports with parents within the 48 hour of the incident time limit.”
“Based on information provided it was found that parents were not notified immediately of an incident where children were able to leave the gated playground area to an open field next to a high traffic side street.”
“Based on information provided it was determined that the operation did not conduct a check of the toddler playground area. This led to a gate being accessible and able to be opened by children and the children were able to leave the secure playground area.”
“At the time of the inspection 1 employee did not have a TB test on file.”
“This standard was evaluated as part of a DFPS investigation and found deficient. Based on information provided, a child with special care needs, that requires additional individual attention to maintain their safety, was left on the playground alone for 5-8 minutes”
“This standard was evaluated as part of a DFPS investigation and found deficient. Based on information provided a child in care was left unsupervised outside on the playground for 5-10 minutes after the class had transitioned into the building. This child has specific special care needs that require individual attention during transition times that the caregivers are aware of.”
“This standard was evaluated as part of a DFPS investigation and found deficient. Based on information provided it was determined that the caregivers responsible for a group of children did not keep track of the children in care and maintain knowledge of the number of children they were responsible for.”
“This standard was evaluated as part of a DFPS investigation and found deficient. Based on information provided a child with special care needs was said to be put to the side in the classroom or outside on the playground and not included as part of day to day activities.”
“This standard was evaluated as part of a DFPS investigation and found deficient. Based on information provided a caregiver did not conduct a face to name accounting of children or maintain the documentation of transitions as required by the operation. This caused the caregiver to be unaware of the number of children they were responsible for and a child to be left outside on the playground.”
“This standard was evaluated as part of a DFPS investigation and found deficient. During pick up times staff are observed cleaning in the classroom which leads to less attention to the children in care as they are being picked up.”
“At the time of the visit 3 of the 10 children records reviewed did not have up to date immunization records.”
“It was found through investigation that a deaf child was left on the playground area after the class had gone inside the building. When conducting the transition the caregiver did not adjust how he accounted for this child based on their specific needs.”
“Through the course of the investigation it was found that a child in care was able to get outside the fence of the playground without the caregivers awareness. When a second caregiver came to the playground area it was found that the child was outside the fenced area and the child was retrieved and returned to the playground area.”
“At the time of the inspection the loose fill on all playground was low and did not meet the required depth and dirt was visible at the base of each slide present.”
“At the time fo the inspection a caregiver was observed helping 2 children with toileting/diapering and did not wash their hands after each instance.”
“At the time of the inspection a caregiver was observed helping 2 children with diapering/toileting and did not have the children wash their hands after each instance.”
“At the time of the inspection the school age class exceeded the maximum capacity for the age group in the class by having 2 caregivers supervising 39 children between the ages of 5 and 12 years old.”
“This rule was evaluated with regard to an investigation and has been found to be in violation. The caregiver did not have visual or auditory awareness of the victim. The caregiver was positioned too far from the victim to intervene.”
“This rule was evaluated with regard to an investigation and has been found to be in violation. The caregiver filled out an incident report after the parent picked up their child. The report was not shared with the parent or director.”
