Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During a DFPS investigation, it was found that bleach water was left in the children's reach.”
“The operation did not notify Licensing when a child was placed at risk when the child accessed bleach water.”
“During a DFPS investigation it was found that caregivers did not properly supervise a child in care when the child accessed bleach water without the caregivers seeing.”
“During an investigation it was found that some of the bottles were not labeled. It was found that sometimes the lids of the bottles are labeled.”
“A caregiver had access to the children in care without a cleared background check. This has been corrected as the person no longer works at the operation. An administrative penalty will be imposed in the amount of 100% of the daily maximum per statute, HRC 42.078(a-2)”
“Observed one child with a food allergy without a signed food allergy form.”
“Observed broken wood pieces, protruding nails, and the sprinkler system hoses exposed which causes a tripping hazard. There were also pieces of broken equipment on the infant playground which was removed from the playground.”
“The operation did not have a current gas inspection.”
“The operation did not know that they had to report positive covid cases. I spoke to the director about reporting any new cases of covid.”
“Observed one infant sitting in a highchair without the safety straps being used. This was corrected when the infant was removed from the chair.”
“During a walk-through of the childcare facility, a 2-year-old classroom was observed to be out of ratio by one child. One child was immediately moved to a different classroom when addressed, bringing the facility into compliance.”
“Three infants not yet ready for table food did not have updated written feeding instructions on file for review.”
“A child sustained an injury while in care that resulted in medical treatment. An incident report was completed but not shared with parent.”
