Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation did not report an incident that required medical attention.”
“One bathroom wall has peeling paint which is accessible to children in care.”
“Operation's last sanitation inspection is dated May 3, 2023.”
“A large squared ceramic tile was observed to be partially detached from the wall/floor area near the children's bathroom sink. The tile has sharp pointed edge that presents safety hazard to children.”
“Two over the counter children's medications and one child's prescription medication that was stored in the main office was found to be expired. NOTE: This was corrected at time of inspection.”
“Two infants were viewed in a buckled chair at a table for more then fifteen minutes.”
“There were 2 infants asleep in bouncers. Then another two infants asleep in swings.”
“A caregiver did not have visibility of two children due to a cubby blocking the caregivers view.”
“Substitute did not know how many children were present upon inspection.”
“Parent was not notified by staff that a injury occurred in their care.”
“There was Lack of good judgment on caregivers due to not keeping the area safe for children.”
“Both staff were sitting down and not actively walking around monitoring children. Both staff also left the classroom with children unattended.”
“A caregiver was seen dragging a child by his feet.”
“Caregivers allowed children to be throwing items across the room. Children were free to do whatever they wanted and were running all in the room.”
“Caregiver failed to count children in her care as they transitioned back to the classroom from outside play. Caregiver did not secure the playground gate after children entered the playground area; the gate leads to a pathway/sidewalk which leads to a roadway with heavy traffic.”
“A child was left unsupervised outside on the playground area while the group transitioned back to the classroom. The child was alone for approximately two minutes.”
“In one classroom, there was a bottle labeled White Rain Kids shampoo & conditioner and a spray bottle of hair detangler sitting on table which was accessible to children in care. There was also a bottle of antibacterial soap in a cabinet under the sink that was accessible to children. In another classroom, two pair of adult size scissors were in a canister atop a desk that were accessible to children. NOTE: This was corrected at time of inspection.”
