Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation has a non-paid background check fee of $4 for invoice #5292600503007QG. The fee invoice was sent to the operation since 9/2/26. Background check invoices must be paid upon receipt.”
“During inspection, a four month old infant was observed sleeping inside a crib. The child was sleeping on his/her back with support from a donut baby pillow. Minimum standard requires for the crib to be bare for an infant younger than 12 months. The operation does not have a sleep exception form available for review to justify the use of the donut baby pillow for this child. Photo taken. This was corrected at inspection after child woke up and the donut baby pillow was removed from the crib. Photo taken.”
“The operation has not completed/submitted a current annual verification of liability insurance. The operation has a past due date of 9/14/26.”
“The operation did not conduct a fire drill for the month of November 2025. There is no documented fire drill. This was corrected at inspection after a fire drill was conducted to bring the operation back into compliance and up to date with their fire drills.”
“Two caregivers did not have current training in pediatric first aid with rescue breathing and choking. A current training card was not available for review during inspection. Their last training card (for both caregivers) expired in October of 2025.”
“The operation has a non-paid background check fee of $2 for invoice #5292500503007A2. The fee invoice was sent to the operation since 9/2/25.”
“The operation did not have a Children's Products Certification available for review.”
“An infant child who is not yet ready for table food did not have updated infant feeding instructions. The last feeding instruction form was signed by the parent on 11/22/23.”
“During inspection, a plastic play structure with a slide was observed to be partially broken from the structure's steps. Broken areas exposed a pinch, crush, and/or shear point. Photos taken. This was corrected at inspection after the play structure was removed from the outdoor play area and thrown away, no longer accessible to children. Photo taken.”
“A child's file did not have a completed enrollment agreement signed by the parent indicating operational policies were shared with the parent.”
“During inspection, a total of 9 infants with a specified age of 12-17 months were observed to be in care in the infant room with two caregivers. The infant capacity on the license/permit is 8. Therefore, the operation was in violation of the infant capacity restriction by one infant child. This was corrected at inspection after one of the parents picked up one of the infant children.”
“Equipment support posts in the outdoor play area were not marked to indicate the depth at which the sand surfacing material must be maintained. Photos taken. This was corrected at inspection after the climbing equipment support posts were marked/labeled to indicate the depth. Photos taken.”
