Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the course of an investigation, it was determined that the operation failed to call 911 when a child received a critical injury.”
“During the course of an investigation, it was determined that a caregiver failed to use good judgement when a child received a critical injury that required medical attention and the caregiver decided to not call 911 which delayed the child receiving immediate medical attention. A caregiver also did not use good judgement when the parent was notified of the injury. The caregiver did not express the extent of the injury of the child to the parent.”
“During the course of the investigation, it was determined that a bathroom door was broken and fell on a child's hand which resulted in a child being injured.”
“Five out of five admission forms lacked the address for the emergency contact.”
“The infant 1 room classroom was out of ratio by one child. There were 5 infants, ages 0m-11m with one care caregiver. The specified age of that group is 4 infants, 0m-11m, to one caregiver.”
“Three out of 10 caregivers lacked the required 24 hours of annual training. One caregiver lacked 7 hours, the second caregiver lacked 23 hours, and the third caregiver lacked 20.5 hours of training.”
“A child was left alone in the three year old classroom.”
“There was a baby walker on the playground.”
“During the course of an investigation, it was determined that a caregiver was holding children by their feet for discipline practices and was throwing rocks at children.”
“During the course of an investigation, it was determined that a caregiver was threatening to lock kids in a closet.”
“During the course of the investigation, it was determined that a caregiver was spanking children, pinching children, and hitting kids in the back of the head.”
“During the course of an investigation, it was determined that a child was left on the playground unsupervised.”
“During the course of an investigation, it was determined that a caregiver left another caregiver with 17-18 children, and their ages were 18m-2 years old.”
“The director lacked current training in child maltreatment.”
“Based on information gathered through an investigation, a caregiver hit a child on the hand and hit another child on the forehead.”
“The last fire drill exit time was 4 minutes. NOTE: This was corrected at the time of inspection when the operation conducted a fire drill under 3 minutes.”
“One out of 10 employee files checked lacked an employee affidavit.”
“Four infant feeding schedules were not updated.”
“The first aid kit was missing the required tweezers. This was corrected at inspection when the director added the tweezers to the first aid kit.”
“In the Toddler class, there was a six seat toddler table. Zero of six children seated at the table were using the required safety straps. This was corrected at inspection when the caregiver strapped each child into their seat as required.”
“One employee record reviewed did not have the required 24 hours of annual training complete within their required annual training year.”
