Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A sampling of children records were evaluated. One child's immunization record was observed incomplete.”
“One employee who transports children had documentation of completion of only 1.5 hours transportation training for last year's annual training.”
“One child's food allergy plan was not signed by the parent.”
“The operation did not have documentation of the rotated menus regarding which date they have been used. This was corrected at inspection as staff dated the menu posted.”
“The tie dye shirts the children wear on the field trips do not have identification listing the name of the child-care center and the child-care centers telephone number,”
“A sampling of children records were evaluated. One child's immunization record was observed incomplete.”
“A sampling of children records were evaluated. One child's record lacked emergency care authorization.”
“This standard was reviewed as a part of an investigation and found deficient. A caregiver did not redirect a child using positive statements. The caregiver grabbed the child quickly by the arm which could have caused injury.”
“This standard was reviewed as part of an investigation and found deficient. Per interviews conducted it was determined a caregiver has not been promptly changing children's soiled or wet diapers.”
“The operational policies were evaluated and lacked written procedures for the promotion of indoor and outdoor physical activity.”
“The operation did not have carbon monoxide detectors installed in the two buildings.”
