Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Supervision was not ensured when two caregivers left sixteen children unsupervised for a minute and twenty-eight seconds.”
“The caregivers did not make good judgment when two caregivers did not communicate with each other, leading to 15 children being left unsupervised.”
“Supervision was no insured when a child was left unattended on the playground for a minute.”
“Two caregivers did not make good judgment when they failed to report to the operation that they left a child unattended on the playground.”
“In one classroom, adult-size scissors, staple removers, and a shipping tape dispenser was observed in an unsecured drawer that was accessible to children. NOTE: This was corrected at time of inspection when the caregiver removed all items from drawer and placed the items out of reach of children.”
“One child's prescribed EpiPen was expired as of July 2023.”
“Based on the information provided during the course of the investigation, one staff failed to use good judgment when she did not count children before leaving the classroom during a transitioning period.”
“Based on the information provided and video footage observed, one staff did not properly supervise the children when lining children up to go outside. As a result one child was left unsupervised in the classroom for 14 minutes while in care.”
“Inspector observed one staff outside the infant room while one staff was with 8 infants 4-12 months inside the room.”
“Based on the information provided during the course of the investigation, there is sufficient evidence to validate that staff used poor judgment when going in and out of a classroom and not ensuring the door was properly closed. As a result a door was left open and a child left the room.”
“Based on the information provided during the course of the investigation, two staff did not properly supervise the children in their care. As a result one child left the classroom on his own.”
“The First Aid Kit was missing the following: Scissors, , Cotton Balls and Thermometer. This was corrected when the Assistant Director placed the missing items in the First Aid Kit. Corrected during the inspection.”
“Based on the information gathered, a caregiver left a child unattended on the playground and didn't know the child was missing until another employee brought the child in.”
“Based on the information gathered, the operation failed to demonstrate good judgment, when the staff did not accurately count children.”
“Based on the information gathered, the operation failed to demonstrate good judgment, when the staff did not communicate with other staff upon shift change. The morning staff didn't explain the injury of a child to the afternoon staff, therefore upon pickup staff couldn't explain injury.”
“Based on the information gathered, the operation failed to demonstrate good judgment, when the staff assumed a child's injury was not severe and did not inform the parent/guardians until the child was picked up from daycare.”
“Based on the information gathered, a school-age child was left napping in a room and unsupervised for approximately 3-4 minutes.”
