Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One out of eight staff records were incomplete as it was lacking the orientation statement, notarized affidavit, pre-employment affidavit, diploma/education verification, pre-service information, etc.”
“An infant was propped up with a padded infant wedge on their side and there was no sleep exeption form on file for the infant. This was corrected at inspection when it was addressed and the padded wedge was removed.”
“One out of eight child records were lacking current immunizations.”
“The emergency practice drills had not been documented since April 2026. This was corrected at inspection when the documentation for emergency practices was updated to include drills practiced from April to August 2026.”
“One out of four employee training records were not available for review.”
“Infant feeding instructions had not been updated in over 30 days for one infant not yet ready for table food.”
“Safety drills have not been documented since April 2025.”
“The operation was not following all field trip requirements when taking children for walks around the neighborhood. Some examples include: children did not wear any identifying information with the operation's name and phone number such as a shirt or nametag; permission slips were not always signed; there was no 48-hour notice posted; emergency consents or contact information was not taken.”
“A caregiver stepped out of their classroom for a very brief moment leaving children unsupervised.”
“The operation was out of ratio when they go on walks outside of the daycare in the neighborhood. There were at least 12 children present ranging in age from 18 months to 4 years old with only 3-4 caregivers present.”
“The operation did not notify Child Care Regulation that they were offering field trips as a service.”
“Based on the preponderance of information gathered, a caregiver used a food item as a reward after a piece of candy was given to children for good behavior.”
“Based on the preponderance of information gathered, a caregiver did not give a child a piece of candy after the child misbehaved. The candy was given to the rest of the children in the class.”
“Re-cite: Four infants were observed to be swaddled while asleep in their crib for naptime. The sleep exception forms on file did not indicate that swaddling was medically necessary. This was corrected when it was addressed and the swaddles were removed.”
“Two classrooms were observed to come inside from outdoor activities and the children did not wash their hands. This was corrected at inspection when it was addressed with the caregivers and the children washed their hands.”
“Two infants were observed to be swaddled while laying in their crib for naptime. The sleep exception forms on file did not indicate that swaddling was medically necessary. This was corrected when it was addressed with the caregiver and the swaddles were removed.”
“The director's record was lacking an annual training topic.”
“The children's products certification was last certified on 01/03/2023. This was corrected at inspection when the form was updated and posted.”

“One caregiver went to open front door and left another caregiver in charge of 2 classrooms which put caregiver out of ratio.”
“A caregiver was concentrating on the negative behavior and not offering praise or encouragement for positive behavior.”
“A caregiver was using language that humiliated the children.”
“One out of ten children's records evaluated lacked a copy of immunization record.”
