Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the inspection multiple medication were past their discard by date and some were expired on the original prescription label.”
“During the investigation it was determined that proper supervision was not met when a child was left unsupervised on the playground for a period of time.”
“During the inspection a caregiver inside a classroom was observed to have an inactive background check.”
“After an investigation it was determined that the operation was aware of water damage to their building on April 27th, 2023, and did not report in the required timeframe.”
“This Minimum Standard was found to be out of compliance through a DFPS Investigation. A caregiver was found to have grabbed a child by the hair and pulled.”
“Proper supervision was not met when a child was left alone in a classroom for around 2 minutes.”
“Good Judgment was not made when two caregivers assumed the other had grabbed a child left alone in a room.”
“During the walk through a leak from the ceiling was noticed in an area that children have access to.”
“During inspection employee files were reviewed and one file was not complete and available for review.”
“During inspection there were three changing pads observed with tears in the pads, the pads could no longer be sanitized properly. The operation corrected the changing pads at inspection.”
“Employee files were reviewed at inspection, one file was missing the notarized affidavit, personnal policies and center policies statement.”
