Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A child's EpiPen had expired in the previous month and was still on site.”
“During the investigation, it was found a child was grabbed and pulled by a caregiver.”
“During the investigation, it was found a caregiver yelled at a child.”
“A caregiver did not use good judgment when giving candy containing vitamins to children in care.”
“At inspection, one caregiver had an expired CPR/First Aid card.”
“A classroom with a specified age of two-year-olds requires a 1:11 caregiver-to-child ratio. At the time of the incident, the classroom initially had two caregivers with 11 children. When one caregiver stepped away, the ratio became 1:12, exceeding the required limit.”
“There were three expired EpiPens. Two from last month and one from October 2024.”
“One infant feeding instructions had not been signed or updated within the last 30 days.”
“One child did not have the 3rd dosage of their PCV immunization on file.”
“The operation was only serving mandarin fruit cups for a snack.”
“There were cleaning supplies and other toxic materials, like sunscreen, left in areas accessible to children. This was corrected at the inspection when the items were moved to higher shelving out of reach from the children.”
“Based on the information obtained, it was determined that a caregiver did not use positive redirection to a child in care. This was corrected when the operation removed the caregiver and made a report to CCR.”
