Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Five children's admission forms did not have a health statement.”
“There was a cot being used by a child with the mesh torn.”
“Three children were missing the complete emergency contact information on their enrollment form. Emergency contact information requires the name, phone number and address that is listed on the enrollment form.”
“The provider conducted two diaper changes and did not wash the toddlers hands afterward. Note, this was corrected when the provider was informed and washed their hands.”
“The permit holder had 3 of 30 required annual training hours completed.”
“The pet dog did not have an up to date vaccination record.”
“One of seven children's files did not have an address for their emergency contact.”
