Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During inspection, a caregiver stated that she had 12 children but had 15.”
“Based on the information obtained, it was determined that a child was left unsupervised at the playground before another classroom caregiver found the child and had the child returned to their class.”
“A caregiver was aware of how many children were in the group at the playground but did not accurately count them all when leaving the playground back to class, resulting in one child being left behind.”
“1 child's immunization exemption form was incomplete, as it was not notarized. The same child still did not have a health statement on file.”
“1 caregiver file did not have verifiable documentation of 24 hours of annual training within the last full 12-months.”
“10 child records were missing admission information, such as emergency contact person address, verification that parents received complete operational policies.”
“6 caregiver files did not have verifiable documentation of all 24 hours of required pre-service training within 90 days of employment.”
“1 child immunization record was not on file. 2 child immunization records were not current. 2 child records did not have health statements.”

“This standard was found deficient as part of a DFPS investigation. The operation's supervision procedures for certain age groups was determined as not sufficient enough to provide visual or auditory awareness and proximity to children during restroom use. Children were allowed to walk from their class to a restroom far enough away from the class where the caregiver was not able to maintain adequate visual or auditory supervision.”
“A child was unintentionally left unsupervised in the gym area for approximately 2 minutes when the rest of the class had transitioned to the outdoor playground.”
“2025 fire drills were not documented, as required. Second half of 2024 fire drills were also not documented. Director stated the fire drills were conducted but not documented.”
“The operation did not report an incident placing children at risk.”
“A child was left unsupervised on the playground at the operation.”
“Operation did not notify parents that a child in care was left unsupervised.”

“10 children's records did not have the required water activity information.”
“10 children's records were not up-to-date with all required admission information.”
“5 caregivers did not have certificates of annual training in: Preventing and responding to emergencies due to food or an allergic reaction Handling, storing, and disposing of hazardous materials”
“An infant under 12 months old was observed swaddled while sleeping in a crib. The infant did not have a sleep exemption form on file.”
“A bottle warner in an infant room needed to be cleaned out, as it had dried formula or water mineral build up.”
“Two caregivers did not have current training in infant care specific training, such as SIDS, Shaken Baby, Early Brain Development.”
