Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The most recent sanitation report is dated 5/2/2025.”
“The first aid kit on the bus was missing anticeptic wipes and tweezers. The first aid kit in a classroom had expired anticeptic wipes.”
“Two employees did not have notarized affidavit forms.”
“Children's product certification was not available for review.”
“The operation did not notify Child Care Regulation of an incident that placed a child at risk of harm.”
“A caregiver did not show self-control when redirecting a child.”
“Supervision was not maintained when a child exited the building through the front door.”
“A one-year-old was left unsupervised for approximately two minutes.”
“An address was not obtained for an emergency contact.”
“The teacher fed the wrong infant formula bottle to wrong child and the bottle belong to another child.”
“Five children's records were reviewed. One child is missing a health statement in their file.”
“Verification of monthly testing of the carbon monoxide detector was not provided. Note: the carbon monoxide detector was tested at inspection.”
“Infant feeding instructions for the seven infants enrolled in care have not been updated every 30 days.”
“An infant was fed approximately 2 ounces of breastmilk from the bottle of another infant in care.”
