Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two caregivers did not know how many children they were responsible for. One caregiver did not know the ages of the children in their care.”
“The operation did not have a current fire inspection during the inspection. The fire inspection was conducted on 7/10/26.”
“Information obtained throughout the investigation shows that a child was left unsupervised outside for approximately 20 minutes.”
“One child's medication was expired.”
“One out of ten children files was missing an immunization record.”
“Information obtained throughout the investigation shows that the operation failed to notify Child Care Regulation within two days of a situation that placed a child at risk.”
“Information obtained throughout the investigation shows that the operation did not share a copy of an incident report with a parent and obtain the parent's signature withing 48 hours of when the incident occurred.”
“Information obtained throughout the investigation shows that a caregiver failed to supervise a child when a child was left alone in a classroom during a transition from the classroom to the gym.”
“Information obtained throughout the investigation shows that the operation failed to notify parents immediately of a situation that placed a child at risk.”
“Information obtained throughout the investigation shows that the operation did not have the parent sign an incident report within 48 hours of becoming aware that the child received medical treatment.”
“It was found that the operation was aware of a situation that placed a child at risk and failed to notify Childcare Regulation of that occurrence.”
“It was found that a caregiver failed to maintain visual and auditory awareness of children's going activities.”
“A child enrolled at the operation was left unsupervised on one of the playgrounds for approximately 5 minutes. During the investigation the classroom was observed and training for caregiver was reviewed and operation has met compliance following the situation.”
“During the investigation it was found that the operation did not notify the parents of an incident involving a child being left on the playground until 3 days after the incident occurred.”
“During the inspection a caregiver did not have documentation of SIDS, Shaken Baby Syndrome, and Early Brain Development.”
“During the walk through there was no documentation for carbon monoxide testing. This was corrected at inspection when the carbon monoxide check was documented.”
“Based on information obtained during an investigation, it was determined that the childcare facility did not report, within the required timeframe, a child injury requiring medical treatment by a health-care professional.”
“Three out of 12 children's files reviewed, needed updated immunization records on file.”
