Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“An infant was observed asleep in a restrictive device upon arrival to the room. The caregiver did not move them out as soon as they could.”
“Ceiling tiles throughout the school are either missing, broken, or stained and need to be replaced. Walls need to be cleaned, especially door frames where handprints build up. Clean A/C filter metal cover to remove building up of dust and dirt.”
“During the investigation it was determined that a child received an injury while in care and did receive medical treatment. The daycare did not report the incident to licensing and complete the correct incident form.”
“During the investigation it was found that a child had sustained more than 10 minor injuries in a 6-month period that were not reported to the parent at pick up and did not have explanations of how the injuries occurred when the parent questioned the caregiver.”
“Four employee records were reviewed and #3 and #4 have cpr/first aid training but need to take the skills part with mannequin.”
“Four staff records were reviewed. Staff #1, #2, and #3 are missing all 6 required topics. Staff #4 is missing food allergies and hazardous materials training.”
“During the inspection multiple ceiling tiles were observed to be hanging out exposing insulation.”
“During the inspection 6 Children's Records were observed, out of the 6, 3 did not have the Special Needs Statement filled out. 2 of the records also did not have the full Emergency contact information and the date of admission.”
