A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“1 out of 4 parent files reviewed, did not have all of the boxes checked for the receipt of the operational policy.”

“A 9 month old infant was observed sleeping with a blanket in a restrictive device and when moved to the play pen. The caregiver removed the blanket after being asked to do so.”
