Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Three of the children's records lacked the name and telephone number of the school where the child attends.”
“Four children's record lacked the name, address, and telephone number of the child's physician or an emergency-care facility”
“Four household members did not have documentation of completed orientation to the childcare home.”
“The child care home was not equipped with a smoke detector in the room being used by children.”
“The primary caregiver only had 21 of the 30 clock hours of training for the previous training year (6/2025 - 6/2026). The remaining 9 clock hours will need to include: emergency preparedness, management, and child development.”
“Documentation was not available to verify monthly checks/testing of the smoke and carbon monoxide detectors and the fire extinguisher.”
“One household member over the age of 14 who is living in the home does not have an active background check.”
“One household member has not completed orientation to the childcare home.”
“Monthly documentation of the smoke detector checks was unavailable for review. This was corrected at inspection when the provider tested the smoke detector and documented the date and time.”
“One child's statement of health from a health care professional was missing from their file.”
“The activity plan or daily schedule was unavailable for review.”
“This standard was evaluated and found to be deficient when the provider had bug spray, lysol spray and wood glue assessible to the children in care. This standard was placed in compliance when the hazards were removed.”
“This standard was evaluated and found to be deficient when the provider did not have documentation of emergency drills available for review.”
“This standard was evaluated as part of an inspection and found to be deficient when two children were in care without health inspections.”
