How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

CCR 101212(d) — Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made…

“Based on the LPA's observations, a written unusual incident report (UIR) for an incident occurring on March 11, 2026 was not received by the Department until March 24, 2026. The facility notified the Deparment by telephone, however by the next working day. This poses a potential health, safety or personal rights risk to persons in care.”

CCR 101416.2(b) — Infant Care Teacher Qualifications and Duties.

“Based on the LPA's interviews with staff and a personnel record review, the facility did not ensure that staff S2-S4 met the qualifications required to work in the capacity of a teacher. Record review indicates that S2-S4 were only qualified to work as an aide under the direct supervision of a teacher. Based on the LPA's interviews, it was corroborated that S2-S4 supervised awake children for a portion of the day without being under direct supervision of a qualified teacher, which poses a potential risk to the children in care.”
