Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on a file review for child C1 performed on 10/21/25, the facility was unable to provide the LPA with child C1's record during the initial investigation visit. The center director, [name withheld] stated that she was unable to locate the contents of C1's file and that no other records were available for review at the time. This poses a potential risk to persons in care.”
“Based on an interview with the acting center director, Anais Heim on 4/11/25, it was corroborated that the facility's former center director, Melissa Terhune is no longer employed by this facility effective 03/14/25. The Department has not been notified of this change. This poses a potential risk to persons in care.”
“Based on interviews conducted and documents obtained, it was corroborated that staff S2 roughly handled child C1 as C1 attempted to sit up in their cot during nap time. This posed a health, safety and personal rights risk to the children in care.”
