How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

Paperwork — records, postings, forms Standard — supervision, ratio, training Health & safety — direct risk to a child

Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

“On June 1, 2026, the Office of Child Care Licensing (OCCL) received a self-report regarding Staff #1 aggressively handling Child #1. Administrator #1 stated Staff #1 was immediately placed on administrative leave and later terminated from employment. On June 8, 2026, OCCL received a complaint regarding a lack of positive behavior supports related to Staff #1 and Child #1. An investigation began on June 1, 2026, and concluded on July 23, 2026. This complaint is substantiated because the details of the center s self-report and the Reporter s allegations allege the same incident, which is Staff #1 inappropriately and aggressively handling Child #1.”

“On January 9, 2023, The Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline. An investigation was conducted on January 13, 2023. On January 18, 2023, an additional complaint was called into the Child Abuse and Neglect Hotline by law enforcement. This complaint was not accepted for investigation by the Institutional Abuse Unit, and was sent to OCCL to be investigated. The complaint is substantiated for improper discipline because the specialist observed video footage showing staff restrained Child #2 to a 12-inch by 24-inch area while seated, in between the staff member s body, a sensory table, and a closed door. This staff member also moved Child #2 by the shoulders after the chair Child #2 was sitting in got placed on the staff members foot. Photographs provided showed red marks on Child #2 s shoulders from being removed from the chair.”

“OCCL received a complaint on August 4, 2022, regarding accident and injury procedures, soiled clothing, and not respecting a child s family. The complaint is substantiated because the child was left in the soiled clothing until the parent arrived to change her son. There is no evidence to substantiate the allegation of improper accident and injury procedures because all staff interviewed denied this and a complete accident report was provided. There is no evidence to substantiate the allegation of Administrator Wendy Deberry-Hill not understanding or respecting the child s family because all staff members stated that they have not experienced Administrator Wendy Deberry-Hill address someone unprofessionally or use a rude tone.”

Substantiated complaint

“OCCL received a complaint on February 25, 2022, regarding improper staff-to-child ratios and improper screen time procedures. An investigation was conducted on March 1, 2022. The complaint is substantiated for improper staff-to-child ratios because on March 1, 2022, unqualified staff were alone in two classrooms, and attendance records for the week of February 22, 2022, documented the morning and afternoon were out of ratio daily and had unqualified staff alone with children. On multiple occasions the staff-to-child ratio was almost doubled. Maximum group size was exceeded on February 22, 2022, and February 25, 2022. A concern noted was the early childhood administrator and curriculum coordinator were no longer employed as of February 9, 2022, and the facility did not notify OCCL of the change. Two staff did not know the proper staff-to-child ratio for the children in their care.”
