Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the information obtained during the investigation, it was determined that the operation was non-compliant with this standard as a caregiver wrote on a child's face with a marker as a form of discipline.”
“At the time of inspection, 4 staff did not have required affidavit in file. This was corrected at inspection by having the staff complete their affidavits.”
“At the time of inspection, 2 staff with a hire date of 3/2024 did not have notarized affidavit in file.”
“The two's classroom had a spray bottle of bleach on a low shelf in the classroom accessible to children. This was corrected at inspection, the staff moved the bleach to a higher counter.”
“February and March fire drills were not documented.”
“1 child's (toddler) file did not have a health statement”
“5 infants in care had expired infant feeding instructions.”
“All classrooms requiring nap equipment did not have the nap mats labeled to each individual child.”
“The two year olds were seen watching an ipad that was not related to activities. The caregiver turned off the ipad and sat the children at the table to complete an activity.”
“A wall in the restroom of the prek classroom was seen with holes.”
“One caregiver's personnel file did not contain a notarized affidavit.”
“Three caregivers did not have a TB test on file.”
