Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“There are 5 children to 1 caregiver with a specified age of 11 months.”
“One play structure at the center had a rope swing.”
“A play structure at the center does not have loose fill or unitary surfacing underneath or surrounding the structure.”
“A platform that was more than 30 inches high did not have a protective barrier preventing the children from falling off of the structure.”
“One set of steps on a play structure did not have a handrail.”
“A caregiver did not demonstrate good judgment and self-control when she aggressively picked up a child and forcefully sat him in a chair.”
“A Review of the Employees Who Left Employment in 2024 data from January 22, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.”
“The operation did not have a copy of the parent's right in the operational handbook. The Director is going to give every parent a copy and have them sign it and then send licensing copies that the parents received the form.”
“The operation did not enter the number of employees who left the operation in 2023. This task was due no later than January 15, 2024.”
“The operational policies were lacking information related to the support of inclusive services.”
“The vehicle that is currently used for transportation is missing electronic child safety alarm.”
“I did not observe the most recent fire inspection available for review.”
“2 out of 6 files reviewed were missing the emergency address for children enrolled.”
