Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“While conducting the walkthrough. The cargiver in the infant classroom was utilizing her cellphone and hid it behind a pillow when CCR staff introduced themselves.”
“A review of the Employees Who Left Employment in 2024 data from January 17, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.”
“During the inspection it was found that the operation had no conducted a severe weather drill since January 2024. This drill is required 4 times per year.”
“During the inspection it was found that the operation had rearranged two of the rooms and it resulted in less indoor space available for use, which reduced the overall capacity in these rooms. One room had 8 children present, but was only enough space for 6 children. The second room had 10 children present, but was only enough space for 7 children.”
“During the inspection it was observed that the television was on for the children during their meal time.”
“During inspection the caregivers did not provide the accurate ages of children in care. Note: corrected during inspection.”
“During follow-up inspection, there was no documentation of director documenting her arrival and departure times at the center.”
“A 13 m child was eating snack with a toddler group. The oldest toddler was almost 3 years old.”
“The operation did not inform Licensing that a serious event took place.”
“There was no incident report created for the situation as required by Minimum Standard.”
“There was evidence gathered that a child was left unsupervised.”
“Based off the evidence founded throughout the investigation, the Director is not routinely at the operation for a minimum of 75% of the time.”
“The emergency preparedness plan does not include how children under 24 months will be evacuated.”
“Based on information obtained during the investigation there is a sufficient amount of evidence to conclude that a child exited the gate at the operation and was found by a passerby.”
“Based on information obtained from the DFPS investigation there is evidence to support a 6 inch space in the fence that allowed a child to exit the playground.”
“Based on information received during the investigation there is sufficient evidence to support that a caregiver did not use good judgment when the caregiver did not immediately retrieve the child from an unknown person after the child excited the playground.”
