A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“The operation did not have available for review (during the inspection) personnel records and annual training hours for two staff.”
“Four children enrolled in care did not have the following information: Authorization to obtain emergency medical care and to transport the child for emergency medical treatment Two out of the four children did not have an address for the child s physician or an emergency-care facility.”
