Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Four of the infant's cribs were not labeled with their names or any additional information.”
“The Toddler and Preschool classrooms did not have written activity plans posted. They were using lesson plan guides provided by Texas Rising Star.”
“There were two out of three infant feeding scheduels that were not up-to-date and signed by the parent.”
“The loose fill underneath the slides was less than 6 inches. The ends of the slides were lifted and did not touch the bottom as I observed. There was 2.5 inches observed of loose fill.”
“The napping cots for the toddlers and Pre-K children were not labeled with their names.”
“The first aid had expired Anitiseptic wipes.”
“One out of six files reviewed did not have a health statement on file.”
“During the investigation, it was found that the staff member who provided first aid was not certified at the time of the injury and had exceeded her ninety days of employment.”
“The loose fill at the base of the sides in the large playground did not meet the required depth of 6 inches; instead, it measured only 1 inch. Additionally, the loose fill at the base of the swings and slide in the small playground was also insufficient, with a depth of just 2 inches.”
“The operation's ten passenger van did not have an alarm installed on the vehicle.”
“Three of ten children's records reviewed for immunizations did not have current immunizations records on file.”
“The fire extinguisher was not secured in two of two vans used to transport children.”
“Two of ten children's files audited did not have a current health statement.”
“One of ten children's files audited did not have current vaccinations.”
“7 out of the 10 children's records reviewed were missing updated immunizations.”
“Three ceiling fans had accumulated dust. Note. This was immediately corrected at inspection by director.”
