Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the investigation and information obtained the operation is not in compliance. A caregiver pushing a child into a chair and toward a table in an aggressive manner.”
“Based on the investigation and information obtained the operation is not in compliance. The caregiver failed to take appropriate action to stop the children from running, which did compromise the safety of a child.”
“Based on the investigation and information obtained the operation is not in compliance. Caregiver in charge demonstrated a lack of good judgment when caring for children and performing her assigned responsibilities.”
“Based on information obtained during the investigation, it was determined that the bus driver slapped the child and pulled the child's ear while on the bus.”
“Based on information obtained during the investigation, a caregiver left a child in the bathroom during the transition to the playground.”
“Based on the preponderance of the evidence, supervision was not met when the caregiver was not walking the playground and supervising the children in all areas.”
“Based on the information obtained ,there was no incident report provided to the parent.”
“Based on the information obtained, the operation did not report the incident to CCR.”
“Based on the information obtained and observation during the investigation, the medicine was at the front desk office and was not in locked storage.”
“Based on the preponderance of the evidence, it is found that supervision was not met when a child swing the belt and hit another child in the mouth with the belt.”

“Based on the information and observation during the investigation, it was determined caregivers were out of ratio. There were 6 infants ages 4 months to 12 months old in the two infants' rooms, with one caregiver for each classroom. The operation is over ratio by 2 infants. Note: Other caregivers to help in these classrooms. Ratios complied during inspection time.”
