Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“3 of the 10 files reviewed the caregivers did not have 24 hours annual training hours.”
“In several classrooms there was not a sufficient amount of space to walk in between the cots during naptime.”
“In two of the classrooms a cot was blocking the entrance of the classroom.”
“According to the video footage the victim was left alone in the gym for approximately 4 minutes. Three caregivers admitted that the child was left alone. A caregiver who was supervising the victim stated that she did not complete a proper name to face count.”
“There was no documentation of the pre-service training completed by a caregiver.”
“A child was left alone on the playground for over 5 minutes.”
“A child was left unattended in a classroom alone.”
“The fencing around the playground is leaning in toward the playground equipment.”
“4 out of 10 files reviewed were missing an address for another responsible individual (friend or relative) who should be contacted in an emergency when the parent cannot be reached.”
“7 out of 10 children files were missing a health statement or the box on the admission form to provide the health statement within 12 month of admission was not checked.”
“A caregiver was in the classroom with children and did not have an eligible background. This was corrected at the inspection when the caregiver was removed from the classroom.”
“This standard was evaluated in regard to an investigation and found to be deficient. A child was left alone on the playground by a caregiver while in care at the operation.”
“1 out of 2 employees and the director do not have current transportation training.”
“The operation did not practice safety drills as required.”
“The director's first aid training is expired.”
“A child with a diagnosed food allergy did not have a food allergy action plan on file at the operation.”
“The operation did not conduct monthly playground checks as required.”
“The fire extinguishers on both buses were expired.”
“Medication that has passed its expiration is kept at the operation.”
“The director's CPR training was expired.”
“This standard was evaluated in regard to an investigation and was found to be deficient. The caregiver used inappropriate discipline when children had toileting accidents.”
“This standard was evaluated in regard to an investigation and found to be deficient. The caregiver yelled at the children when she became frustrated.”
“A caregiver was caring for children without a complete background.”
