Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the inspection, the carbon monoxide detector was found to have dead batteries and did not activate when tested.”
“During the inspection, the operation was unable to produce documentation verifying monthly testing.”
“The standard was assessed during an investigation and found to be deficient. A child was able to leave a classroom and enter a different classroom alone without caregiver knowledge.”
“One child's file that was reviewed did not have the consent information needed for proper medical treatment.”
“One child's file did not contain authorized release information.”
“The handbook did not provide the essential information required for parents.”
“Two children's files did not include the address for their emergency contact.”
“A child with a documented food allergy did not have a signed authorization form from their physician.”
“The documentation provided indicates that the center has not been completing its weekly and monthly maintenance outdoor checks.”
“Three children's case files that were reviewed did not include the necessary information regarding a physician and/or hospital as required.”
“Your Health/sanitation inspection expired on 1/24/2025.”
“This standard was evaluated during the course of an investigation and was deficient. A child was left unsupervised in the classroom for 15 minutes.”
