Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One classroom was missing a emergency evacuation diagram at inspection. *It was corrected at inspection*”
“The fire inspection was not current at inspection.”
“The operation did not have the carbon monoxide detector documented as completed for the past month.”
“During the investigation, it was determined the operation did not notify the parent of a less serious injury at pick up. *The operation had an incident report written but did not provide it that same day.*”
“A caregiver did not know the number of children in her care when asked.”
“A few of the medication forms were not consistent with the full name of the staff administering the medication. This was corrected when a staff had to administer medication and wrote full name.”
“The opening room was observed with the TV on, and two children were under the age of two. This was corrected when the teacher turned the tv off.”
“A disinfecting solution was in an unlock bottom cabinet accessible to children. This was corrected when the teacher put it in the bathroom up high on a shelf.”
“Three out of ten children's records were missing the address for the emergency contact. This was corrected when the director called parents for this information.”
“Dust buildup was observed in some of the toy buckets in the afterschool room. Note, that this was corrected when the buckets were cleaned. The fan in the afterschool room was observed to have an accumulation of dust on the blades. This was corrected when the fan was cleaned. A cleaning solution was observed on a changing table accessible to children. This was corrected when the bottle was moved. The air conditioner vent was observed to have an accumulation of dust. This was corrected when the vent was cleaned. Cleaning supplies were found in an unlocked cabinet accessible to children. This was corrected when a lock was put on it.”
“Transportation Van with Lic plate #JYK 7093 did not have an operating child safety alarm.”
“5 out 10 children files were missing required information. Emergency Contact must have name, phone number and an address so that person may be reached at any time.”
“The operation failed to submit the number of employees that left their employment in 2023 by the January 15th deadline.”
“Based on the information gathered two children were playing in the kitchen center unsupervised.”
“Based on the information gathered three children were left unsupervised in a classroom.”
