Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Caregiver in the two-year-old classroom was supervising children during their naptime while playing a barnyard game on their cellphone. Note, this was corrected when the staff was informed that they could not be using their cellphones for personal use when supervising children. Director created a form reminding staff of the cellphone policy and had all staff signed off on it.”
“The AM toddler caregiver did not communicate to the PM caregiver before ending their workday of a child that sustained a scratch.”
“During the course of this investigation, it was determined a child's care services were terminated for filing a complaint.”
“There was a delay in communication between a caregiver and parent to inform them that their child had an altercation with another child earlier in the day and resulted in a scratch to their face.”
“An incident report was created, but not shared with the parent. The child is no longer enrolled.”
“There was play equipment structure that had pieces of flooring that were breaking causing pinching points. *Dir is having the equipment removed*”
“Three out of ten records reviewed did not have all of the required trainings complete at review.”
“A caregiver did not know the number of children in their care when asked.”
“An infant was a sleep in a bouncer seat. *This was corrected when the caregiver placed the infant in the crib on their back.*”

“Two televisions were observed on low shelves not anchored.”
“Four employees did not have an updated SIDS training.”

“This form for the 5 employees records evaluated was not available for review.”
“A playground checklist was not available for review. Some areas of the playground did not have enough surfacing due to rocks needing to be racked in the areas. A water hose was out and accessible to children as a tripping hazard. A couple of fences were broken and unable to be closed. Two metal poles were missing covers and were sharp to the touch.”
“Through the investigation, it was determined that there were 24 children to one caregiver. The group's specified age was five years old. Making the group out of ratio by two children.”
“At the time of inspection, there were six infants in the classroom with one caregiver, ages 12 to 17 months. This was corrected when one child unter17 months was moved for the infant room.”
“Two employee files reviewed did not have current SIDS and Shaken Baby Training.”
“Two employee files reviewed did not contain current abuse / neglect training.”
“There were two broken boards on the playground accessible to the children in care.”
