Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One child who has been enrolled for over 12 months did not have a signed health statement from the physician on file.”
“The children's files evaluated did not have signatures that parents recieved Parent Rights information.”
“Four of seven files did not have signed medical authorizations for children in care.”
“Three of seven children's files did not have addresses for each in case of an emergency contact.”
“One of seven children's files did not have an up to date immunization record.”
“Two children lacked the health statement.”
“The children's product form has not been re-certified. The form was last certified 1/13/2020.”
