Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver washed their hands after changing a diaper in a sink with a bottle that was being warmed up in water.”
“A caregver did not demonstrate good judgement when changing another student in close proximity of another while toileting.”
“A child ran out of the bathroom after toileting and went to play with other children without washing hands.”
“The operation has not had a valid city fire inspection since July of 2021.”
“Six infants that are not on table food did not have updated feeding instructions withing 30 days.”
“Loose fill around climbing structures in the outdoor toddler play area and preschool/pre-k play areas was observed to be kicked out around bases of slides and below the required depth of 6 inches.”
“At the time of the inspection a group of 17 children, ages 12 to 22 months were mixed during the nap period. This group exceeded the maximum group size for children ages 12 to 17 months by 4 children.”
“At the time of the inspection the bricks on the back of the wall for the dumpster container had partially fallen and bricks were in the playground area. The wall was able to be moved and appeared unstable.”
“Based on information obtained through the investigation it was found that caregivers were unable to maintain supervision of children in care while children on the play structure; which led to children pulling clothing items off during play.”
