Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Operation was found to have acted cruel and using an unusual punishment by placing a 3-year-old child in a crib in the infant room as punishment.”
“Director failed to allow time for a child to eat after they were served breakfast.”
“Caregivers did not execute good judgement by allowing an employee to place a 3-year-old child in a crib and not remove them after knowing it was incorrect.”
“Director failed to operate in compliance of the minimum standards that they themselves must enforce, by taking food away from a child and ridiculing a 7-year-old child for having a sipping cup.”
“Based on information gathered during a DFPS investigation, it was determined that a caregiver grabbed a child causing them to hit their head on a wall.”
“Based on information obtained during a DFPS Investigaton, it was determined that parents were not notified of less serious injuries at pickup.”
“Based on information gathered during a DFPS investigation, it was determined that a caregiver yelled at children at the operation on multiple occasions.”
“Based on information gathered during a DFPS investigation, it was determined that a parent was not notified immediately of an incident that involved a non-routine situation that placed the child at risk for harm.”
“Based on information gathered during a DFPS investigation, it was determined the operation did not share an Incident/Illness Report with the parent within 48 hours.”
“A caregiver was unaware that a child left the group to go inside to the restroom. Another caregiver found the child alone in the classroom.”
“Mattress cover in the crib was too small and did not fit mattress.”
