Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Operation failed to contact Child Care Regulation after there was an incident regarding a caregiver grabbing a child.”
“Caregiver used prohibited punishment when they grabbed a child and yanked them at the operation.”
“Operation failed to have proper supervision when caregivers allowed a child to walk around the playground, chewing a weed (grass) in their mouth, which became stuck in their throat.”
“Caregiver yelled at a child while supervising a classroom.”
“Four out of five infants requiring feeding sheets did not have updated forms in the last 30 days. This was corrected at inspcetion when parents updated forms.”
“Caregiver observed using cell phone for personal use during nap time.”
“This was evaluated during a DFPS investigation and found to be deficient.”
“This standard was found deficient as part of the DFPS investigation. A four year old child was left unsupervised in a public restroom on a field trip. Child was found by an individual of the public and returned to group. Caregiver did not take steps to ensure child was accounted for and was unaware child had separated from the group. In addition, two other children in the visited a second restroom without caregiver supervision.”
“During a DFPS investigation, it was determined that the operation was out of ratio during a field trip. There were children ranging in age of four years to twelve years of age and the operation failed to meet the 8:1 ratio requirement.”
“Based on a DFPS investigation, it was determined that staff failed to properly document their field trip transition/checklist sheets when accounting for children.”
“One food allergy plan was not signed by the parent.”
“A caregiver used their hand to strike and push a child's forehead for no apparent reason. The caregiver has is no longer employed by the operation.”
