Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two out of eight children files did not have updated immunization records to review.”
“Medication was not in the orginal box and not labeled with child's name.”
“Four of ten children's records did not have current immunizations records on file.”
“Two of nine staff did not have the required 24 hours of annual training.”
“Three of ten staff records did not have an affidavit on file for review.”
“Medication that was on file for a child enrolled at the operation was expired.”
“The food allergy emergency plan that was on file for one of the children enrolled was not signed or dated by the child's physician.”
“One of child's emergency file was incomplete. The address was missing for the emergency contacts listed. This was corrected at inspection when parent updated the information at pick up. The addresses for both emergency contact were added.”
